Flex on Chip or Flex-rigid prototype

Any recommendations for chip-on-flex protype providers?
I want a flexible substrate or a rigi-flex substrate. I also want the QFN chips mounted using SMT . Is there such a prototyping service offered in US? What price points are considered reasonable?
