B9176030 Pin Layout for Gate Control Board, Testing Resistors & Where to Buy New IC

This B9176030 used in gate control board it seems had some problems, no relay action after push test button on the board. I'd like to test the resistor between each pin and ground, who have the pin layout? Who knows where can I buy a new IC?
