You'd test hi-lo, lo-grd, hi-grd most of the time. I would usually use the red lead on the portion that is being tested. If you want, you can guard out whichever element you want to measure. For instance, for hi-lo, connect your guard terminal to the frame of the transformer, so the measurement is of the insulation between the hi side windings and low side winding only.
