    Include Benchmark chart with HDtune, with before and after AVG removal, just keep whatever you can in the background.

    According to disk damaged. Let others comment, but since you booted the system 100% from a USB pen, there should be no such "teeth".
    There should be flat "steps" like in my screenshot at the end (non-sterile graph).
    Perhaps the scan in MHDD will confirm it, maybe not ...

    Ubuntu itself (eg internet) with USB pen works flawlessly and without jams?

    Check in MHDD (and show the screen) how the disk responds to the command:
